When people reach out to me, they are usually concerned about a healthcare situation or a decline in functioning and are looking for professional support and guidance understanding and accessing care options. I understand that each person’s situation is different and there may be variables that need to be considered when developing an appropriate care plan. I offer different levels of services in both Care Advisory and Long Term Care Insurance consulting. Below you can see the details of each.
Because of numerous variables that impact each personal situation, I start every inquiry with a 15 minute complimentary discovery call to see if your situation is something I can assist with and to offer input as to which service might be right for you.
This package includes a 75 minute in-home comprehensive care assessment with customized recommendations, resources and referrals. Upon scheduling this appointment you will be asked to complete an initial intake prior to the scheduled meeting. Included in this package is a 30 minute follow up session (phone call or zoom) scheduled 1-2 weeks after the initial consultation.
This package includes a 60 minute call (phone call or Zoom) to review written recommendations that will be created after the initial discovery call. This might include specific resources and referrals tailored towards your unique needs. Written recommendations, referrals and direct introductions to providers will be made if desired.
This 30 minutes call (phone or Zoom) will address specific questions and concerns about your situation. This may be used if the Comprehensive Care Management Consultation and/or Care Management Resource Call don't fit your situation. During this session, we will focus directly on your unique questions and concerns, leveraging my professional knowledge, extensive resources, and elder care network to guide you through your specific circumstances.
*Some Long Term Care (LTC) insurances offer reimbursement for Care Coordination/ Care Advisory services. Typically these benefits are only reimbursed once the claimant is approved under the LTC claim. Payment is expected from the client at time of scheduling services. If you are found to be eligible for reimbursement of these services under your LTC benefits we will submit necessary invoices and credentials required for reimbursement of services rendered.
This is a 60 minute consultation that can occur in my office or over Zoom. This package is designed for the client who already has a LTC insurance policy or is considering purchasing LTC insurance and would like a comprehensive overview of the policies benefits, eligibility and nuances. This session offers ample time to answers any specific questions you may have. Clients must upload LTC policy documents when scheduling this service so they can be reviewed before the scheduled consultation. Included in this fee is an additional 30 minute follow up session if needed,
This package is best suited for the person who would like to initiate a long term care claim but would like to strategize a plan to proceed. During this 2 hour in-home care assessment I will offer customized resources and referrals tailored to your specific needs. In addition we will contact you LTC carrier (with claimants verbal approval) to clarify the policies current benefits, provisions and eligibility requirements. If deemed appropriate a long term care claim will be initiated during this session. A Comprehensive Care Advisory Evaluation to support the LTC claim will be emailed to you 5-7 business days of our initial session. Two additional 30 minute follow up calls are included in this package.
This is for the person who wants us to do it all! This package includes a 2 hour in-home Comprehensive Long Term Care Evaluation, LTC Policy Review, Initiation of LTC Claim, referrals to appropriate providers and community resources. Written Care Advisory Report to be submitted in claims documentation. Additional visit or coaching in preparation for the LTC carriers on site evaluation to determine eligibility of claim. Follow up with current and previous care providers to ensure documentation and invoicing of their services are applied towards the LTC policy elimination period if applicable. Credentialing of all independent care providers and entities to ensure eligibility. Weekly follow up with LTC insurance until a determination is made. Submission of all invoices to ensure reimbursement. Ongoing follow up with claimant and family by phone regarding progress of claim.
Monthly home visit to monitor ongoing care needs and make additional referrals to care providers if necessary. Oversee care providers documentation and services in place to ensure compliance with care plan. Submission of monthly invoices to LTC claims if applicable. Ongoing communication and emotional support via phone or email with claimant or designated family members regarding care services or concerns.
Please Note:
Travel time will be billed at a rate of $75/hour and will be invoiced after the completion of the work.
Additional home-visits made after the designated service agreement will incur an additional fee of $175.00.
